AUTHORIZATION AND CONSENT TO RELEASE INFORMATIONTO METHODIST WELFARE SERVICES (MWS)I hereby irrevocably and unconditionally authorize Methodist Welfare Services (MWS) and/or its third party service provider appointed for this purpose, to perform background/ reference checks and otherinvestigative actions and enquiries in review and assessment of my application of employment. In addition, I have been informed and understand that the collection, use and disclosure of such personal data and/or information will be used to verify the accuracy of information that I have provided to MWS in my Employment Application Form and/or curriculum vitae as deemed necessary at the organization’s discretion.I further consent to the collection, use and disclosure as well as compilation and/or reproduction of any or all my personal data and/or information discovered or received on me into reports or documents, whether in written or electronic form, for MWS’ reference and consideration. I also understand and acknowledge that this consent form shall be valid until I withdraw my consent in writing to MWS.I hereby authorise any persons or entities who may be contacted to provide information relevant to these enquiries to disclose it to MWS. I release all such persons or entities requesting or supplying such information from any and all liability on account of such disclosure whether under the Personal Data Protection Act 2012 and general rules on confidentiality or any other similar law or regulation from time to time.I further consent that a photocopy of this authorisation be accepted with the same authority as the original.
