Job purpose
The Medical Social Worker works closely with Agency for Integrated Care (AIC) on referrals from hospitals and government-linked agencies and other relevant stakeholders to support aging in place for patients and their families in the community. The incumbent is responsible to support a coordinated approach with multi-disciplinary stakeholders to regularly review bio-psycho-social-spiritual aspects to ensure holistic care.
Duties and responsibilities
· To provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
· Participate in multi-disciplinary team meetings in developing individualised care plans and facilitate family conferences for patients/clients and family members together with members of the multi-disciplinary team for facilitation of effective communication
· Assess caregiver stress and burden and provide supportive platforms and initiatives for caregivers
· Assessment of risk and to intervene in a timely manner
· Information and referral services for patients and family members as deemed necessary for patient/client’s optimal functioning in the community with relevant resources
· Undertake projects and any other duties as assigned by Reporting Officer or Head of Centre/Centre Manager
